Welcome to the online nomination form for The World Food Prize. Please use this form to submit information on your nominee. Please click "SIGN UP AND SUBMIT YOUR PLAN" above to begin the submission process. If you have already signed up and begun your submission application, please "LOGIN" to access your submission form.

INSTRUCTIONS:

Upon entering the site, you will create a user name and password, which you can use to access your nomination at any time.

After creating your password, you will be asked to complete three steps to fill out the nomination form. You must complete each step before you will be allowed to progress to the next. You may save your progress at any point and return later to complete the nomination using your login information.

STEP 1: NOMINATOR INFORMATION

You will be required to fill out these fields:
Nominator Name
Current Position (Title)
Organization
Phone number
Email
Mailing Address

STEP 2: NOMINEE INFORMATION

You will be required to fill out these fields:
Nominee Name
Current Position and Organization
Date of Birth
Country of Origin
Synopsis of Achievement (150 words or less)

You will be required to upload these separate documents:
Nominee Resume or CV
Photo of nominee
Statement of Achievement
Statement of Impact

STEP 3: DOCUMENTATION

You will be required to upload these separate documents:
Nomination letter from the nominator
2 seconding nomination letters
Up to 5 additional documents (optional)


All submissions are due by May 1. To submit the nomination, you must click the button "SUBMIT MY NOMINATION". Upon submitting the form, you should see a confirmation message.
